Historical Background Among the responsibilities given the Superintendent of the Poor was to provide a record of deaths occurring at the Warren County Almshouse to the New York State Department of Health. Instructions for completing the two-part document (Certificate of Death and Report of Death) were very specific, even to the point of providing appropriate detail as as to the cause of death. Indeed, the documents warned that unsatisfactory death certificates would not be accepted! And while individuals were advised that all entries must be completed in their entirety, omissions were not uncommon!
Description of Record Series Preprinted booklets containing the stub portion of the Report of Death and containing: name, place and date of death, age, color, family information, cause of death, certifications, date and place of burial. Total # of reports: 151. Not all information completed on all report stubs. CO-2 Series # 633.
Purpose of Record A record of death of inmates of the Warren County Home (Almshouse) as held by the Superintendent of the Poor with original information and certificate of death reported to and filed with the local Registrar of Vital Records.
