Long Term Home Health Care Program
The Long Term Home Health Care Program is New York State Department of Health certified and provides medicaid eligible clients who qualify for skilled nursing facility care. Focus is to provide a cost-effective comprehensive alternative to nursing home placement for those who prefer this option.
There are 18 different services available through the Long Term Home Health Care Program. Services provided to clients in the program are based upon medical needs but can include:

- Physical Therapy
- Occupational Therapy
- Speech Therapy
- Nutrition
- Medical Social Worker
- Respiratory Therapy
- Telehealth
- Audiology
- Medical Model Day Care
- Home Health Aides
- Personal Care Aide
- Homemaker
- Social Day Care
- Lifeline
- Home Delivered Meals
- Respite Care
- Transportation
Skilled nursing is the only direct service provided by the agency in this program. All other services are provided on a contractual basis which necessitates a full-time coordinator on a supervisory level to be sure all services are timely and appropriate, as well as, good coordination between all the services a client receives.
