       We wish to welcome you to our services and we hope that your experience with us is a positive one.

       Warren County Health Services is a county sponsored home care provider certified by the state and federal government to provide skilled services to you a resident of Warren County. We have many services and programs available. The skilled services we provide are Nursing, Home Health Aide, Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Worker, and Nutritionist. The disease management programs include Chronic Heart Failure, Chronic Obstruction Pulmonary Disease, Diabetes program along with pain management, Dyspnea (difficulty breathing) protocols and hip and knee replacement protocols. We also have a Long Term Care Program (“Nursing Home Without Walls”) for those clients who are eligible medically and financially. Another service that we provide to the community is our Central Intake assessment, where a nurse and social worker, assist clients and family members in identifying home care needs and directs them as to where to obtain the services.

       In 2004 Warren County Health Services was recognized by IPRO (Experts in Defining and Improving the Quality of Health Care) for “Outstanding Service to the Health Care Community” for “Home Health Quality Improvement”. Only four agencies in New York State were recognized. In 2005 New York State Department of Health conducted our re certification survey and the agency received no deficiencies.

       As a certified agency, we are able to bill your insurance for our visits. The nurse or therapist will discuss any skilled services that are not covered by your insurance that you might need at the evaluation visit. A fee schedule based on income is available to you for any uncovered services. We encourage and invite your input in developing an appropriate and realistic plan of care.

       Medicare as well as some other insurance companies requires you to be homebound in order to receive home care services. The intent is that you are unable to leave the home safely to receive skilled services. Medicare clarifies homebound, as “the condition of these patients should be such that there exists a normal inability to leave home or if able requires considerable and taxing effort. Absences from the home are infrequent, or for periods of relatively short duration or are attributable to the need to receive medical treatment.”

       Our staff is required by state and federal regulations to assess your homebound status with every visit. When there is a change in your homebound status, your insurance carrier and your physician will be made aware. It is not our intent to stop services to you based on homebound status as long as there continues to be a need for skilled services. The agency has a fee schedule based on income for skilled services. No person will be denied needed skilled services however based on inability to pay. We will be happy to discuss these options with you or your family if and when the need arises.

       We welcome your comments and suggestions at anytime during your admission and a satisfaction questionnaire will be sent to you after discharge for any additional ideas, concerns, or comments.
