Goals of the Agency:
Provide instruction and support to patients and families in disease management and self-care through the use of skilled nursing services, therapies, medical social services, nutritional, home health aide services and telemedicine.
Provide health guidance to all ages to help individuals, families and the community achieve and maintain optimum health.
Participate in ongoing assessment of the community's health, social needs, and resources to affect appropriate program planning.
Quality Improvement Program
Program is designed to improve and enhance client outcomes through monitoring.
Program consists of 9 components including established Plans of Action, client and physician satisfaction surveys, as well as chart and focus audits to name a few.
The Quality Improvement Team consists of administrative and clinical staff representatives from nursing and physical / occupational / speech therapies, medical social workers and nutrition.
