Cooperative Efforts With Other County Services
The agency has made a commitment to ensuring easy access to health care in Warren County. In an effort to meet this commitment, skilled nursing services have been made available to the Department of Social Services and Office For The Aging in the following programs.
PCA - Personal Care Aide Program (Department of Social Services)
This is a medically funded program administered by the Department of Social Services providing personal care aides to qualified clients. The aides assist with personal care, shopping, and housekeeping chores. Our Agency nurses perform the skilled assessment needed to determine eligibility and also provide the supervision of the circle once services are provided.
EISEP - Expanded In-Home Services for the Elderly Program (Office For Aging)
A state funded program through Office For Aging that provides aide assistance in the home with personal care, and / or housekeeping chores to clients who meet the program's criteria and financial requirements. Regular nursing assessments are done to supervise the aide and manage the ongoing services.
Central Intake Home Care Needs Assessment:
Agency nurses work jointly with a Department of Social Services Case Worker doing in home assessments for individuals who request
assistance in determining long term care needs. This highly skilled team assesses the medical and financial needs and identifies
sources available to assist them and will make referrals to these sources when appropriate.
This team also is certified to do PRI / SCREENS (forms required for nursing home placement) and will assist individuals and families in the nursing home placement process.
