Considerations for Placement
In order for an individual to be considered for placement at Countryside Adult Home, the following requirements must be met:
- Independent with ambulation (walker, cane, or wheelchairs are acceptable, as long as the individual is able to propel their own wheel chair);
- Independent with transferring in and out of a chair, on and off the bed, etc.;
- Continent of Bowel/Bladder or able to manage your incontinence;
- Diabetics - need to be able to perform their own glucoscans, inject their own insulin, and read their sliding scale, if necessary;
- Oxygen - If an individual is on oxygen, they need to be able to manage such on their own (change tubing, filters, water, switch from tank to concentrator, adjust settings on equipment, etc.);
- Do not pose a risk to themselves or others;
- Do not have an active Medicaid case in another county, or have an agreement for another county to cover their cost of stay;
- Are not at risk to wander;
- Have a current Medical Evaluation completed prior to admission (can be obtained from Director), as well as a PPD no more than 30 days prior to admission.
